How can I view my referral?

You can only view your doctor's referral when you have received an e-mail from your referrer, usually your general practitioner. If you have not received an email, please ask your referrer to verify your email address and resend the email.

Viewing your referral

1. Click Bekijk uw verwijzing in email

• If this button does not work, use the link below the blue button

2. Click the blue button Stuur sms-code

3. You will now receive a code per sms. Enter this code and click Inloggen. When you are logged in, you will find information on your referral and appointment
